YEARLY REQUIREMENTS FOR STUDENT-ATHLETES

Returning Student-Athletes at BC
All student-athletes must complete forms annually in order to be cleared by the Sports Medicine staff to practice and compete. Please note the required forms below that both first-time and returning BC student-athletes are required to supply. If you have any questions, please contact the Sports Medicine staff.Â
First-Time Student-Athletes at BC
University Health Services – Some of this information can be completed on your phone/tablet/computer through your Health Services portal. However you will need to upload your physical exam, immunization records form, and sickle cell trait test results into your Health Services portal.  Health Services will not accept any hard copies of these forms; all documents must be uploaded via the portal. Any questions related to uploading Health Services forms should be directed to 617-552-3225.
- Boston College Health History
- Physical Exam from the student-athlete's primary care physician (PCP)  – Note that this must be completed within 6 months prior to enrollment. This form will need to be uploaded. Use the "Document Upload" drop down.
- Immunization Form - This form will need to be uploaded. Use the "Document Upload" drop down
- Immunization Dates -Â In addition to uploaded your immunization form you need to electronically enter all immunization dates. Use the "Immunization" drop down
- Information and waiver form for Meningococcal Vaccination (2 pages) -Â If you have received the meningococcal vaccine you do not need to complete this form
- Tuberculosis Screening/Testing Form (3 pages)
- Concussion Education Form

- Sickle Cell Trait testing results - These results need to be uploaded. All children born in the United States are tested for sickle cell trait at birth. International Students may not have been tested for sickle cell trait at birth and many families in the US no longer have the birth testing results. If this is the case, the athlete will have to specifically request this testing from their physician (Sickle Cell testing is not part of normal blood work). Documentation from hospital, physician's office or lab results are all acceptable.  It generally takes at least a week to obtain testing results—please plan appropriately when scheduling this testing.

Sports Medicine – All forms must be completed in the student athletes' Teamwork's profile. These forms will be assigned to each student athlete in their Teamwork's profile. Sports Medicine will not accept any hard copies of these forms. Any questions related to the sports medicine forms should be directed to 617-552-4540.
- Insurance Information Letter – parent/guardian signature required. Keep a copy for future reference.Â
- Intercollegiate Insurance Form
- Photocopy of the front and back of insurance card
Returning Student-Athletes
All forms must be completed in the student athletes' Teamworks and University Health Services portal. Sports Medicine will not accept any hard copies of these forms. Any questions related to the sports medicine forms should be directed to 617-552-4540.
We would like to specifically stress that parents/guardians carefully read the information presented in the INSURANCE INFORMATION LETTER. Â This document explains the insurance requirements, our policies regarding payment of bills, and explains what to do when you receive medical bills at home that are directly related to an athletic injury. Â And as noted, please keep a copy of this letter for reference during the year.
Health Services – Please use the Health Services portal link to access the student athletes Health Service portal. These forms will be located under the "Forms" section of their portal.Â
- Athletic Interim Physical Questionnaire – The student athlete will need to complete this form before vitals can be taken. Sports medicine staff will work with coaches and student athletes on when the vitals will be taken based on when they arrive back to Boston College's campus this summer/fall.
