## Sports Medicine: Yearly Requirements for Student-Athletes

All student-athletes must complete forms annually in order to be cleared by the Sports Medicine staff to practice and compete. Please note the required forms below that both first-time and returning BC student-athletes are required to supply. If you have any questions, please contact the Sports Medicine staff.Â

First-Time Student-Athletes at BC
- Sickle Cell Information (PDF)
- Copy of the front and back of health insurance card(s) - MANDATORY
- A completed physical exam and immunization records from the primary care physician and a family health history. The physical should take place in the six (6) months prior to the beginning of classes. Health Services forms can be found by visiting Boston College Health Services. These forms will be sent in a University mailing and returned to Health Services. Questions regarding these forms should be directed to Health Services (617-552-3225).
- A signed copy of the Insurance Information LetterÂ

- A completed Intercollegiate Insurance Form. Fax copies do not print legibly and will only be accepted as a temporary form.

- A completed NCAA Banned Substance Medical Exception Form**Â with supporting information to the address listed on the form
- ** This form needs to be completed annually by your physician ONLY if the student-athlete has been diagnosed with ADD or ADHD and has been prescribed medication for it. If the student-athlete has not been diagnosed with ADD or ADHD, then this form does not need to be completed.
Returning Student-Athletes
